The Herbert Protocol is a simple but really important form that can make a huge difference if someone with dementia ever goes missing.
It records key information such as:
- A recent photo
If that person is reported missing, this information can be shared immediately with police and partner agencies, helping officers search faster and more effectively at a time when every minute matters.
The form is completed in advance by the person themselves, a family member, friend or carer, and kept safe until it’s needed.
If you support, care for, or know someone with dementia, we strongly recommend completing a Herbert Protocol form.
