I emailed you on 11 October regarding you wanting to set up your new scheme.  

There were a number of questions we needed answers to before we could approve your scheme.  However,  I have not received a reply.

Please could you come back to me with the information required as we are keen for your scheme to be approved so that you can start receive useful and supportive information.
